Position Title:Case Manager

Job Description:

Under direction, determines applicants’ eligibility for a variety of social services available and/or provides other social services related to meeting basic household needs, prevent homelessness, educational and training for a workforce solution to promote sustained employment and case management. Interviews clients, collects and completes required eligibility documentation, determines eligibility for social services assistance and monitors client progress and usage of assistance. Refers clients to external clinical/social service provider resources; and performs related duties as required.

Responsibilities:

  • Interviews clients, collects and completes required documentation and determines eligibility for social services assistance based upon a varied of service regulations and procedures
  • Conducts in-depth assessments of client needs by providing basic counseling and case management services for clients;
  • Develops casework/service plans and monitors progress by conducting follow-up sessions
  • Coordinates with other social service agencies, organizations, and vendors to provide appropriate services and information to clients
  • Serves as an advocate on behalf of the client by making appropriate referrals to other social service, education, and health agencies as needed
  • Plans, organizes, and participates in special needs projects including outreach efforts, public speaking, and special activities to clients and the Bexar County community
  • May perform related duties as required

Requirements:

  • Graduation from an accredited college or university with a Bachelor’s Degree in Psychology, Counseling, or Social Work with three (3) years of experience in social services related work and; or an equivalent combination of education and experience
  • Knowledge of social service policies/procedures and of local social services, organizations, programs, and resources available; knowledge of Federal, state and local programs laws and requirements; knowledge of the legal system as it relates to the mentally ill; knowledge of computer equipment to include word processing, spreadsheets, databases and a variety of software packages
  • Skill in providing assistance to clients and referring clients to appropriate social services organizations and programs; skill in interviewing techniques; skill in problem solving and decision making
  • Ability in dealing with persons experiencing mental health and economic stresses; ability to communicate effectively with hostile, abusive or irrational individuals; ability to work effectively under high-pressure circumstances; ability to communicate effectively, both verbally and in writing; ability to communicate effectively with diverse groups of individuals utilizing tact and diplomacy; ability to operate personal computer and basic office equipment; ability to establish and maintain effective working relationships with clients and/or families, co-workers, County employees, and community outreach and other organizations, clinical and other social service providers
