Job Description:

At Leicester City Council, we are changing the way we deliver adult social care and support services.  We work within a wider context of individual’s own abilities, family, and wider community support in a strength-based approach. We work very closely with our Health colleagues in the Integrated Locality Teams to maintain the health and wellbeing of people in Leicester is a shared responsibility.

Responsibilities:

  • As a Care Management Officer, you’ll receive referrals, carry out assessments, monitor and review cases, provide appropriate information, advice, and services to those making contact with the Department.

  • You’ll receive initial contacts and referral for adult’s social care, whilst providing an effective response to emergency situations.

  • You’ll undertake assessments and arrange services to meet identified needs in line with the Care Act, whilst ensuring that the provision of services are appropriate to the needs of a multi-racial, multi-cultural and multi-lingual population.

  • Using your experience of working with adults with a range of social care, educational, health and housing needs, you’ll confidently work with the adults and carers to assess/review needs, plan support to meet these needs, and identify strength-based outcomes.

  • With a commitment to working in partnership you’ll show the ability to work within a multi-disciplinary team particularly with specialist staff from other agencies such as Health, Probation, and Housing.

  • Due to the nature of this role you’ll need to be registered with Social Work England, hold a valid driving UK licence, and will also be required to show proof of being doubly vaccinated against Coronavirus. This post is subject to an enhanced DBS check.

Requirements:

  • You’ll need an ability to use basic IT programmes e.g. Word, Email, Client Database, along with sharp written communication skills to produce good correspondence on a variety of subjects.

  • Using your experience of working with adults with a range of health issues you’ll have the ability to create innovative appropriate packages of support to maximise independence whilst minimising risk.

  • You’ll demonstrate strong interpersonal, communication and organisational skills and the ability to work within a multi-disciplinary team.

  • The service is busy and suits someone who is energetic and efficient, able to pay attention to detail and can consider safety and risk issues quickly and confidently.
